WHAT DO I NEED TO DO?

  1. Contact us on 02 9906 7777 or email office@archealth.com.au you can also make a referral via our website.
  2. Have your basic information ready including your NDIS number, your Plan Manager and the contact details of either your Support Co-ordinator or Local Area Co-ordinator (LAC) details.
  3. All of our referrals come through our Operations Manager Sue Sutton and we can organise that you discuss your case with a therapist if your needs are highly complex. We can contact you on your preferred number and we can guide you in the right assessment and treatment pathway.
  4. Once we know we can help you, we will send you a welcome email and Service Agreement and a link to complete our admission form.
  5. Please read, complete, sign and return the Service Agreement confirming you have sufficient funds to cover the cost of your initial therapy assessment. The cost of this assessment, including any travel, is included in the Service Agreement. *
  6. If you have a support co-ordinator, a copy of your signed Service Agreement will be sent to them for their records.
  7. We will also book in your initial therapy assessment either at your home or our clinic in St Leonards.
  8. Your therapist will provide you with a Treatment Plan during or following your initial assessment. Please make sure you fully understand their recommendations and the cost associated with them. You will be asked to approve the Treatment Plan before it commences.
  9. A copy of your approved Treatment Plan will be sent to your Support Co-ordinator and to your Plan Manager so that they can confirm that you have sufficient funds and can create a Service Booking to lock away those funds.
  10. If you do not have sufficient funds available through your NDIS plan, your therapist will work with you to modify the treatment plan based on the funds available, or you have the option of paying for some sessions privately.
  11. After each treatment session, your therapist will generate an invoice that will be sent to your Plan Manager for payment.

 

*Please note, if you are unable to sign and return the Service Agreement to us, our therapist can collect if from your home during your assessment or this can be signed prior to your assessment in our clinic

  8. We will also book in your initial therapy assessment either at your home or our clinic in St Leonards.
  9. Your therapist will provide you with a Treatment Plan during or following your initial assessment. Please make sure you fully understand their recommendations and the cost associated with them. You will be asked to approve the Treatment Plan before it commences.
  10. The treatment plan will be used by our admin team to secure NDIS funds in a Service Booking to cover the cost of all sessions agreed on the plan.
  11. We will let you know if there are insufficient funds in your NDIS plan as shown on the portal and your therapist will discuss with your how the treatment plan can be modified to use the funds that are available.
  12. A copy of your approved Treatment Plan will be sent to your Support Co-ordinator if you have one for their records.
  13. After each treatment session, your therapist will generate an invoice that will be claimed by our admin team through the NDIS portal.

  8. On the completion of your initial assessment, your therapist will generate an invoice which will be emailed to you for payment.
  9. Our payment terms are 7 days from date of invoice.
  10. Your therapist will provide you with a Treatment Plan during or following your initial assessment. Please make sure you fully understand their recommendations and the cost associated with them. You will be asked to approve the Treatment Plan and check that you have sufficient NDIS funds available for the whole treatment plan, before it commences.
  11. After each treatment session, your therapist will generate an invoice that will be emailed to you for payment.

 

*Please note, as self managed clients you are expected to follow our 7 day payment policy. If timely payment does not occur, a late payment fee may be added
